You should consider filing a complaint with the Washington State Department of Health if you believe that the actions of a provider were inappropriate or may have caused mental or physical harm to you or someone else. Reporting potential violations helps to protect the people of Washington.
The Washington State Department of Health will investigate reports involving fees or insurance claims only when there appears to be fraud involved. The Department may not be able to help you with many common complaints unrelated to patient safety such scheduling problems, personality conflicts, or disputes over bills or insurance.
